OPP screening form that needs to be completed for all team officials (coaches, ass’t coaches, trainers, managers, etc.).  The screening needs to be completed annually and even if you completed one earlier this year for some other purpose (coaching soccer, baseball, etc.), a separate screening needs to be completed specifically for KDMHA.

Process to follow:
 
Complete the top portion (name, dob, gender, address, etc.) and in the box of ‘Name of Organization’ write ‘Kemptville and District Minor Hockey Association’.  Sign and date and that’s it – the box of ‘Name/Title’ and line of ‘Signature – Organization Witness’ can be left blank.  She does require one piece of photo id.
 
If you can get to the station during business hours (8:30am-4:30pm), she will do it while you wait – only takes a few minutes. Confirm before going (258-3441).
 
If you cannot get to the station during business hours, follow these steps:

1. Complete and sign the form.
   
2. Photocopy your license or another piece of photo id with your name etc. (enlarge and make sure it is completely legible).
   
3. Have someone take the form and id photocopy to the station for you.
   
4. You will be contacted once the screening is completed.
   
5. Someone else may be able to pick-up the completed form for you. Carol asks that you call to confirm before going (258-3441).
